Moments Hospice is seeking a Team Director – Hospice (RN Clinical Manager) to lead an interdisciplinary team and ensure high-quality, compassionate end-of-life care in the field. You will mentor clinicians, coordinate with families and physicians, and maintain compliance.

Qualifications

• Active Registered Nurse (RN) license in the state you work in (Wisconsin)
• Proficiency with electronic medical records (EMR)
• Leadership or management experience is a major plus
• Strong understanding of the hospice philosophy and end-of-life care
• Strong organizational, communication, and critical-thinking skills
• A compassionate, patient- and family-centered approach
• Valid driver's license and reliable vehicle with current insurance

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, bereavement coordinators, and volunteers
• Provide clinicians with real-time guidance, mentoring, and clinical problem-solving support
• Lead interdisciplinary group (IDG) meetings and ensure plans of care are accurate, current, and well documented
• Conduct clinical record reviews, case reviews, and home visits as needed to ensure quality care
• Ensure compliance with Medicare hospice regulations and maintain survey readiness
• Manage staff scheduling while fostering a positive, collaborative team culture
• Develop and update patient care policies and emergency preparedness procedures
• Serve as the central point of communication between patients, families, physicians, other providers, and the care team
• Maximize resource utilization to support efficient, high-quality care
