Moments Hospice is seeking a Team Director – Hospice (RN Clinical Manager) in the St. Louis area. You will lead an interdisciplinary team of nurses, social workers, chaplains, and volunteers to deliver compassionate, high-quality end-of-life care.

You will mentor clinicians, coordinate care with families and physicians, and ensure compliance with hospice regulations. The role offers a fleet car program and strong leadership access as Moments Hospice grows.

Qualifications

• Active RN license in the state you work in.
• Proficiency with EMR systems.
• Leadership or management experience a major plus.
• Strong understanding of hospice philosophy and end-of-life care.
• Excellent organizational and communication skills.
• Compassionate, patient- and family-centered approach.

Responsibilities

• Lead and oversee an interdisciplinary team of clinicians and volunteers.
• Provide real-time guidance, mentoring, and clinical problem-solving support.
• Chair IDG meetings and ensure care plans are accurate and current.
• Perform chart reviews, case reviews, and home visits to ensure quality care.
• Ensure Medicare hospice regulatory compliance and survey readiness.
• Manage staff scheduling and foster a collaborative culture.
• Develop/update patient care policies and emergency procedures.
• Serve as the central liaison between patients, families, physicians, and the care team.
• Maximize resource utilization for high-quality care.

Responsibilities

• Lead interdisciplinary group (IDG) meetings and ensure plans of care are accurate, current, and well documented
• Conduct clinical record reviews, case reviews, and home visits as needed to ensure quality care
• Ensure compliance with Medicare hospice regulations and maintain survey readiness
• Manage staff scheduling while fostering a positive, collaborative team culture
• Develop and update patient care policies and emergency preparedness procedures
• Serve as the central point of communication between patients, families, physicians, other providers, and the care team
• Maximize resource utilization to support efficient, high-quality care

Qualifications

• Active Registered Nurse (RN) license in the state you work in
• Proficiency with electronic medical records (EMR)
• Leadership or management experience a major plus
• Strong understanding of the hospice philosophy and end-of-life care
• Strong organizational, communication, and critical-thinking skills
• A compassionate, patient- and family-centered approach
• Valid driver's license and reliable vehicle with current insurance
