Moments Hospice in Minneapolis, Minnesota is seeking a Team Director – Hospice (RN Clinical Manager) to lead an interdisciplinary team delivering compassionate end-of-life care. You will mentor clinicians, coordinate with families and physicians, oversee plans of care, ensure regulatory compliance, and promote high-quality, patient-centered outcomes.

This is a full-time, field-based leadership role with a fleet car program and competitive compensation.

Qualifications

• Active RN license in the state of practice.
• Experience leading interdisciplinary teams in hospice or similar setting.
• Strong knowledge of hospice philosophy and end-of-life care.
• Ability to mentor clinicians and coordinate care with families.

Responsabilités

• Lead an interdisciplinary team of nurses, aides, social workers, chaplains, bereavement coordinators and volunteers.
• Provide real-time guidance, mentoring, and clinical problem-solving support.
• Lead IDG meetings and ensure care plans are accurate and well documented.
• Conduct chart reviews, case reviews, and home visits to ensure quality care.
• Ensure Medicare hospice compliance and survey readiness.
• Manage staff scheduling and foster a collaborative team culture.
• Develop and update patient care policies and emergency procedures.
• Serve as central communication between patients, families, physicians, and care team.
• Maximize resource utilization to support high-quality care.

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, bereavement coordinators, and volunteers
• Provide clinicians with real-time guidance, mentoring, and clinical problem-solving support
• Lead interdisciplinary group (IDG) meetings and ensure plans of care are accurate, current, and well documented
• Conduct clinical record reviews, case reviews, and home visits as needed to ensure quality care
• Ensure compliance with Medicare hospice regulations and maintain survey readiness
• Manage staff scheduling while fostering a positive, collaborative team culture
• Develop and update patient care policies and emergency preparedness procedures
• Serve as the central point of communication between patients, families, physicians, other providers, and the care team
• Maximize resource utilization to support efficient, high-quality care

Qualifications

• Active Registered Nurse (RN) license in the state you work in
• Proficiency with electronic medical records (EMR)
• Leadership or management experience a major plus
• Strong understanding of the hospice philosophy and end-of-life care
• Strong organizational, communication, and critical-thinking skills
• A compassionate, patient- and family-centered approach
• Valid driver's license and reliable vehicle with current insurance
