Qualifications

• One year of experience as a Medical Case Worker I or a Bachelor's degree with one year of professional casework experience.
• Strong data analysis, problem solving, and MS Office proficiency.
• Ability to collaborate with multidisciplinary professionals and partners.

Responsibilities

• Oversees a team of Case Managers and provides supervision to link clients to housing and services.
• Reviews client outcomes and addresses gaps in program delivery and documentation.
• Develops tailored pathways to vital resources like mental health care and housing assistance.
• Promotes coordination of care across multiple partners and manages program growth.
