Job summary

Unity Care Group is seeking a Case Manager for Minors and Non-Minor Dependents in the Transitional Housing Program to deliver direct services and intensive case management for TAY aged 16 to 21. The role collaborates closely with an interdisciplinary team to align individualized goals with community resources.

The position emphasizes eligibility, documentation, and service planning under THP guidelines, with focus on education, employment, housing, and independent living skills for youth.

Qualifications

• Bachelor's degree in counseling or social work with licensure content for LPCC.
• Alternative degree in human services or behavioral science plus relevant experience.
• CCL approval may substitute start date requirements.

Responsibilities

• Provide direct client services and supervision in THP.
• Coordinate care on an interdisciplinary team.
• Maintain weekly contact, goals, and community referrals.
• Complete Medi-Cal paperwork and maintain accurate records.
• Assist with planning, budgeting, transportation guidance, and housing goals.

Skills

Bachelor's Degree in Counseling/Social

Two years' experience with TAY 16–24

CCL approval as Excepted Case Manager

Microsoft Office proficiency

Education
