Job summary

Five Keys Schools and Programs seeks a Transitional Age Youth (TAY) Care Coordinator to provide housing-focused coordination for San Francisco clients aged 18–27. You will support housing applications, navigate benefits, and coordinate services with city programs, ensuring stable housing and progress toward self-sufficiency.

You will maintain detailed records, collaborate with partners, and lead group life skills sessions to empower youth while upholding trauma-informed, equity-centered

Qualifications

• High school diploma or equivalent required
• Bachelor's degree preferred
• Minimum five years of direct service experience with similar population

Responsibilities

• Assist TAY participants with securing housing and navigate housing applications
• Explain tenant rights, lease responsibilities, and requirements
• Coach on timely rent payments and landlord communication
• Develop case management plans and connect to community resources
• Deliver services to stabilize housing, mental and physical well-being, and self-sufficiency
• Coordinate referrals to increase housing stability, income, and job placement
• Collaborate with rental assistance provider to determine appropriate assistance amounts
• Lead Life Skills sessions on budgeting, housing search, nutrition, and tenants’ rights
• Maintain comprehensive client records and regular reports
• Ensure compliance with Five Keys and HSH data standards
