Job summary

Many Mansions is seeking a Case Manager to provide on-site intensive case management for residents transitioning from homelessness in Thousand Oaks and Fillmore. You will assess needs, implement service plans, and coordinate with partners to promote housing stability and wellbeing.

The role requires strong documentation, collaboration with property management, and experience with Housing First principles. Driving may be required in a fast-paced, mission-driven environment.

Qualifications

• Substantial education and/or experience in supportive housing, mental health, social services, or counseling.
• Knowledge of Housing First, Harm Reduction, Motivational Interviewing, and Trauma-Informed Care preferred.
• Excellent documentation and record-keeping abilities.

Responsibilities

• Provide on-site intensive case management for an assigned caseload transitioning out of homelessness.
• Conduct assessments, implement and monitor Individual Service Plans, and crisis intervention.
• Coordinate with community partners to meet housing, health, and income needs.
• Maintain compliant records and data in paper and HMIS databases; ensure privacy.
• Collaborate with Property Management to support eviction prevention and resident success.
