Job summary

Hope the Mission in Thousand Oaks, CA is seeking a Housing-Focused Case Manager to provide housing-first, low-barrier, trauma-informed case management for participants in interim housing, with a clear emphasis on rapid permanent housing placements.

You will conduct assessments, develop Housing Action Plans, maintain accurate HMIS records, coordinate move-in support, and collaborate with housing partners to shorten shelter stays while helping participants achieve lasting stability and

Qualifications

Bachelor's degree in Social Work, Human Services, Psychology, or related field.
Minimum one year in homeless services or housing navigation/case management; or three years with homelessness experience.
Knowledge of Housing First, Harm Reduction, Trauma-Informed Care, and motivational interviewing.

Responsibilities

Provide housing-focused case management using Housing First, low-barrier, and trauma-informed approaches.
Conduct participant assessments to identify housing barriers and eligibility for resources.
Develop and update Housing Action Plans with measurable goals for permanent housing.
Maintain regular contact with participants on housing progress and motivation.
Support tenants in understanding rights, leases, and tenancy expectations.
Assist with housing navigation activities: applications, referrals, documents, interviews, viewings, signings, move-in.
