Job summary

PATH (People Assisting The Homeless) seeks a Case Manager for the Metro Villas SHS site in Los Angeles, CA. The role focuses on helping clients break the cycle of homelessness through individualized service planning, housing stability, and coordinated access to health and social services.

The position supports clients across street outreach, interim, transitional and permanent housing with a client-centered approach, utilizing trauma-informed care and motivational interviewing to promote housing

Qualifications

• Two years of experience in healthcare, case management, homeless services or working with vulnerable populations.
• All levels of education and experience are welcome.

Responsibilities

• Conducts intake and individualized needs assessment and develops ISP with housing goals and income plans.
• Provides referrals and connections to resources to meet ISP goals.
• Completes program assessments (e.g. VI-SPDAT).
• Maintains documentation and participant files per PATH policies.
• Provides crisis intervention and trauma-informed support to clients.
• Maintains knowledge of community resources to connect clients to wrap-around services.
