Job summary

Urban Care Community Limited is seeking a highly skilled Self-Employed Support Worker to provide direct care for individuals with learning disabilities, ASD, mental health disorders and other complex needs. You will develop personalised care plans, assist with medications, and collaborate with families to tailor support to each person.

The role requires empathy, strong communication, and the ability to adapt to evenings, weekends or overnight shifts.

Qualifications

  • Minimum 2 years of experience supporting individuals with learning disabilities and related needs.
  • Knowledge of behavior support, medication assistance, and crisis intervention.
  • Ability to build rapport with individuals, families and colleagues.
  • Ability to work independently and adapt to changing needs.

Responsibilities

  • Provide direct care and support to individuals with LD, ASD, MH disorders, and other needs.
  • Develop and implement personalised care plans in collaboration with families.
  • Assist in medication administration and maintain accurate records.
  • Apply intervention strategies to manage challenging behaviors and promote well-being.
  • Create a safe, inclusive environment supporting physical, emotional, and social health.
  • Collaborate with multidisciplinary teams to ensure holistic care.
  • Maintain up-to-date documentation including progress notes and incident reports.

We are currently seeking a workforce requirement for a highly skilled and compassionate Self-Employed Support Worker/ Carer who possesses experience and expertise in managing individuals with Learning Disabilities, Autism spectrum disorder (ASD), Mental Health Disorders, Communication Disorders, Attention-Deficit/Hyperactivity Disorder (ADHD), Physical Disabilities, Challenging Behaviour, Epilepsy and Diabetes Management.

Key Responsibilities:
  1. Provide direct care and support to individuals with Learning Disabilities, Autism Spectrum Disorder (ASD), Mental Health Disorders, Communication Disorders, Attention-Deficit/Hyperactivity Disorder (ADHD), Physical Disabilities, Challenging Behaviour, Epilepsy and Diabetes Management.
  2. Develop and implement personalised care plans in collaboration with the individual and their families, ensuring that their unique needs and preferences are considered.
  3. Assist in the management and monitoring of medication administration, adhering to prescribed protocols and maintaining accurate records.
  4. Employ effective intervention strategies to manage challenging behaviors and promote positive outcomes for the individuals in our care.
  5. Foster a safe, supportive, and inclusive environment to promote the physical, emotional, and social well-being of our clients.
  6. Collaborate with a multidisciplinary team, including medical professionals, therapists, and educational staff, to ensure the holistic care and development of each individual.
  7. Maintain accurate and up-to -date documentation, including progress notes, incident reports, Challenging behaviour chart, ensuring compliance with regulatory standards and organisational protocols.
Required Skills and Qualifications:
  1. Minimum of 2 years of experience working with individuals with learning Disabilities, Autism Spectrum Disorder (ASD), Mental Health Disorders, Communication Disorders, Attention-Deficit/Hyperactivity Disorder (ADHD),Physical Disabilities, Challenging Behaviour, Epilepsy and Diabetes Management
  2. Knowledge and understanding of best practices in supporting individuals with complex needs, including behavior management techniques, medication administration and crisis intervention.
  3. Strong interpersonal and communication skills, with the ability to build rapport and establish effective relationships with individuals, families and co-workers.
  4. Ability to work independently, prioritise tasks and adapt to changing environments and needs.
  5. Hold relevant certifications or qualifications in related field is preferred but not required.
  6. Empathy, patience and a genuine passion for working with individuals who have diverse needs.
  7. Flexibility in working hours, as the role may require occasional evenings, weekends, or overnight shifts.

Experience:
