Job summary

InReach is seeking a full-time Support Coordinator (Qualified Professional) in the Charlotte/Mecklenburg area to develop and coordinate Plans of Care for individuals with IDD. You will meet with individuals and families, work with community partners, and contribute to a supportive, inclusive environment.

The role offers a Monday–Friday schedule (8:30am–4:30pm) with possible after-hours and on-call work; pay ranges from $50,000 to $55,000 based on experience, with benefits including Medical,

Qualifications

• 4-year degree in human services field is required.
• 2 years of supervised direct experience with the IDD population.
• 1–2 years of management/supervisory experience preferred.
• Innovations Waiver and 1915i services experience preferred.
• A valid driver’s license is required.
• Access to reliable transportation 24 hours a day.
• Safe driving record.

Responsibilities

• Supervise Community Outreach staff in implementing supports for individuals served.
• Monitor, manage, and facilitate day-to-day operations.
• Complete all documentation in a timely manner.
• Implement and review each customer's Plan of Care.
• Plan and oversee work schedule; approve payroll hours.
• Provide one-on-one supervision as needed.
• Share on-call service for emergencies.

InReach has an exciting opportunity to join our team as a full-time Support Coordinator (Qualified Professional) in the Charlotte/Mecklenburg area that is responsible to develop and coordinate Plans of Care for individuals with Intellectual/Developmental Disabilities. Will include meeting with IDD individuals and families to address needs and concerns; must also be willing to work cooperatively with community partners as well as staff from other InReach departments.

Schedule: Monday-Friday 8:30am to 4:30pm *hours may vary depending on customer needs; nights and weekends and on call may be necessary.

Pay Rate: $50,000 - $55,000 based on experience
