Job summary

InReach is seeking a full-time Support Coordinator (Qualified Professional) in the Charlotte/Mecklenburg area to develop and coordinate Plans of Care for individuals with IDD. You will meet with individuals and families, coordinate with community partners, and oversee daily operations including documentation and Plan of Care reviews.

Qualifications

• 4-year degree in the human services field.
• 2 years of supervised direct experience working with the IDD population.
• 1-2 years of management/supervisory experience preferred.
• Innovations Waiver and 1915i services experience preferred.
• Possess a valid driver's license.
• Access to reliable transportation 24-hours a day.
• Safe driving record.

Responsibilities

• Supervising Community Outreach staff in the implementation of supports to individuals served.
• Monitoring, managing, and facilitating operations on a day-to-day basis.
• Responsible to complete all documentation in a timely manner.
• Implementation and review of customers Plan of Care.
• Plan and oversee work schedule, approve payroll hours.
• Provide one-on-one supervision as needed.
• Share on-call service for emergencies.
