Job summary

St. Mary's Home in Norfolk, VA seeks a qualified Support Coordinator (QIDP) to lead the ISP process and coordinate person-centered services for individuals with developmental and physical disabilities.

You will collaborate with families, DSPs, nursing, therapy, and other interdisciplinary staff to ensure plans are implemented, monitored, and revised to achieve meaningful outcomes. Requires relevant licenses or degrees and prior service-planning experience.

Qualifications

QIDP requires one of the licenses or degrees listed above.
At least 1 year of experience with individuals with developmental and physical disabilities.
Background in developing and implementing service plans.
Strong communication, organizational, and multitasking skills.
Experience with electronic health records (EHRs) preferred.

Responsibilities

Lead the ISP process and ensure plans are understood, implemented, and evaluated.
Collaborate with individuals, families/guardians, Residential Services, DSPs, and clinical staff.
Coordinate admission planning and comprehensive assessments for placement suitability.
Develop and maintain person-centered plans with measurable outcomes.
Document ISPs, progress notes, and required reports for departments.
Provide training and coaching to staff on ISP implementation and active treatment.
Attend IEPs or school/day program meetings to advocate for individuals.
