Job summary

Outward is seeking a proactive Support Coordinator to empower adults with learning disabilities, autism, and mental health needs through person-centred support and coordinated service delivery. You will work with the Deputy Manager and Team Manager to ensure safe, well‑organised services and high‑quality care across multiple settings.

Qualifications

• Good understanding of safeguarding and risk management within supported living settings.
• Knowledge of health and safety in a 24-hour supported living environment.
• Experience maintaining records and using digital care systems.

Responsibilities

• Provide direct, person-centred support and promote independence.
• Coordinate day-to-day service delivery, rostering, and risk assessments.
• Lead on shift coordination, provide guidance to staff, and liaise with families and professionals.
• Carry out routine medication checks and contribute to monitoring service delivery.
• Ensure accurate documentation and contribute to service development and improvements.
