Job summary

Care Lync is seeking a Service Coordinator to support adults with disabilities and seniors in navigating housing, medical, and community resources. You will develop individualized service plans, advocate for participant choice, and coordinate with medical professionals and family members to ensure high-quality care.

You will work in a collaborative, growth-minded environment with opportunities for leadership and ongoing training.

Qualifications

Bachelor's degree in Social Work, Psychology, Human Services, or related field.
Strong organizational and communication skills.
Willingness to travel up to 50% to meet participants.

Responsibilities

Conduct person-centered assessments to identify needs and goals.
Develop and update ISP to ensure appropriate services.
Provide participants with choice and advocacy in care options.
Coordinate with medical professionals, behavioral specialists, families, and resources.
Monitor safety, satisfaction, and progress toward independence.
Assist individuals with disabilities and aging population in accessing community resources.
Maintain documentation, confidentiality, and compliance with standards.
Participate in ongoing training and professional development.
