Job summary

Care Lync is seeking a dedicated Support Coordinator to empower adults with disabilities and seniors to live independently. You will assess needs, develop ISP, coordinate services, and advocate for choices in housing, medical care, and community resources.

Travel up to 50% to participant homes and communities, with strong mentorship and professional development opportunities as part of a collaborative, inclusive team.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Comfort working independently and traveling up to 50% to meet with participants in homes and communities.

Responsibilities

• Conduct person-centered assessments to identify participants' needs, preferences, and life goals.
• Develop, implement, and regularly update ISP to ensure participants receive the right services.
• Provide participants with choice and advocacy, helping them make informed decisions about providers and care options.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver seamless, high-quality care.
• Monitor and evaluate services to ensure participants' safety, satisfaction, and progress toward independence.
• Support individuals with disabilities and aging population in accessing community resources.
• Maintain documentation, confidentiality, and compliance with all program and ethical standards.
• Participate in ongoing training and professional development to strengthen your skills and impact.
