Job summary

Care Lync is seeking a Support Coordinator in Maine to assess participant needs, develop ISP plans, and advocate for access to housing, medical care, and community resources. The role emphasizes collaboration with families and professionals to deliver person-centered, high-quality services.

You will coordinate with medical and behavioral specialists, monitor progress, and support participants and aging individuals in achieving independence.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Comfort traveling up to 50% to meet with participants in their homes and communities.
• Valid driver's license, auto insurance, and reliable transportation.
• Commitment to maintaining required certifications and state clearances.

Responsibilities

• Conduct person-centered assessments to identify participant needs, preferences, and life goals.
• Develop, implement, and update Individual Support Plans (ISP).
• Provide participants with choice and advocacy in care options.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources.
• Monitor and evaluate services for safety, satisfaction, and progress toward independence.
• Assist adults with disabilities and aging population in accessing community resources.
• Maintain documentation, confidentiality, and compliance with standards.
• Participate in ongoing training and professional development.
