Responsibilities

• Conduct person-centered assessments to identify needs and goals.

• Monitor and evaluate services for safety and progress.
• Support individuals with disabilities and aging population in accessing resources.
• Maintain documentation and ensure confidentiality and compliance.
• Participate in ongoing training and professional development.
