Job summary

Care Lync seeks an Eligibility Specialist / Case Manager to provide comprehensive case management for adults with IDD, autism, and physical disabilities. You will develop individualized care plans, broker services, and monitor outcomes to ensure cost-effective, quality supports.

The role emphasizes coordinating with providers, ensuring participant choice, and maintaining confidentiality across health information. Requires a BS/BA and at least 3 years of related experience.

Qualifications

• BS/BA degree in related course work.
• Experience with Microsoft Office and state-maintained databases.
• CPR/First Aid certification and updated state disclosures.

Responsibilities

• Perform assessments to determine participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan for each participant and update annually or with changes.
• Coordinate with care teams to ensure ISP supports are aligned with participant goals.
• Maintain confidentiality of sensitive health information and complete required trainings.
• Travel up to 50% to participant sites as needed.
