Job summary

Care Lync is seeking an Eligibility Specialist / Case Manager to develop and implement individualized care plans, coordinate services, and monitor outcomes for adults with disabilities enrolled in Medicaid Waiver programs. The role requires at least 3 years of case management experience, proficiency in Microsoft Office, and a valid driver's license.

Travel to participants is expected up to 50%, and the incumbent will ensure confidentiality and compliance with approvals and policies.

Qualifications

• BS/BA degree in related coursework.
• Minimum of 3 years related experience providing case management for individuals with disabilities.
• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and updated state-related clearances.
• Valid Driver's License including personal vehicle insurance and current vehicle registration.

Responsibilities

• Complete assessments to identify each participant's needs and desires for service delivery.
• Develop a Person-Centered Service Plan for each participant and modify annually or with significant changes.
• Ensure participant choice of providers with information to make an informed decision.
• Ensure consistency of Behavioral Support Plan and Crisis Plan with the ISP.
• Actively coordinate with other care coordinators to ensure seamless services.
• Maintain confidentiality of sensitive information and participate in trainings.
