Care Lync in Brewer, ME is seeking a Support Coordinator to help adults with disabilities and the aging population access housing, medical care, community programs, and needed supports. • Coordinate with medical professionals, behavioral specialists, family members, and community resources.
• Monitor and evaluate services for safety, satisfaction, and progress toward independence.
• Support adults with disabilities and aging participants in accessing community resources.
• Maintain documentation, confidentiality, and compliance with program standards.
• Participate in ongoing training and professional development.
