Job summary

Care Lync in Maine seeks a dedicated Support Coordinator to help adults with disabilities and aging participants access housing, medical care, and community services. You’ll assess needs, develop Individual Support Plans, and advocate for participant choices in a compassionate, person-centered approach.

This role involves home visits up to 50% travel, strong organizational and communication skills, and collaboration with medical and community resources.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Strong organizational, communication, and advocacy skills.
• Willingness to travel up to 50% to meet participants in homes and communities.

Responsibilities

• Conduct person-centered assessments to identify participant needs, preferences, and goals.
• Develop, implement, and regularly update Individual Support Plans (ISP).
• Provide participants with choice and advocacy to select providers and care options.
• Coordinate with medical professionals, behavioral specialists, family members, and community resources.
• Monitor and evaluate services to ensure safety, satisfaction, and progress toward independence.
• Support individuals with disabilities and aging participants in accessing community resources.
• Maintain documentation, confidentiality, and compliance with program standards.
• Participate in ongoing training and professional development.
