Job summary

Easterseals Louisiana is seeking a Support Coordination Supervisors to oversee delivery of coordinated services for participants. You will review plans, contact new linkages, assist coordinators with ISR corrections, and monitor quality of service delivery.

The role requires a relevant degree and two years of experience, strong communication, MS Office proficiency, and the ability to multitask and meet deadlines; you will coach staff and ensure compliance with standards.

Qualifications

• Bachelor’s or Master’s degree in one of the listed fields.
• Two years of paid post-degree experience in coordination services.
• Ability to multitask and meet deadlines.
• Excellent written and verbal communication skills.
• Proficiency in Microsoft Office Software.

Responsibilities

• Review Comprehensive Plans of Care (CPOCs) for regulatory submission.
• Make initial contact for new linkages.
• Assist coordinators with ISR corrections and GPSORC forms.
• Review 10% of case records for completeness and quality.
• Conduct weekly face-to-face case reviews with coordinators.
• Attend weekly staff meetings and manage caseloads.
• Accompany coordinators on annual home visits.
• Provide training and coach staff to meet outcomes.
• Ensure ongoing quality delivery of services.
• Review timesheets and mileage reports for accuracy.
