Qualifications

• Bachelor’s or Master’s Degree in Social Work (CSWE-accredited) with two years post-degree experience in providing coordination services.
• Bachelor’s or Master’s Degree in Nursing (RN) licensed in Louisiana with two years post-degree coordination experience.
• Degree in a Human Service related field with two years post-degree coordination experience.
• B.A. in Liberal Arts with 16 hours in psychology/education/related fields and two years of coordination experience.

Responsibilities

• Complete quarterly visits and monthly contacts, with observation and monitoring.
• Conduct and complete annual CPOCs per funder timelines.
• Plan, schedule, and coordinate quarterly participant/provider meetings.
• Respond to participant changes, submitting required paperwork.
• Research, access, and link needed services for participants.
• Coordinate with providers to ensure appropriate service provision.
• Maintain all responsibilities regarding critical incidents.
• Attend weekly supervision and staff meetings; participate in trainings.
• Support participants during emergent situations and weather-related evacuations.
• Adhere to mandated reporting requirements per Louisiana Children’s Code.

Education

Bachelor’s or Master’s Degree in Social Work (CSWE-accredited) with two years post-degree experience

Bachelor’s or Master’s Degree in Nursing (RN) currently licensed in Louisiana

Bachelor’s or Master’s Degree in a Human Service related field

Bachelor’s Degree in Liberal Arts with relevant concentration
