Qualifications

• Bachelor’s or Master’s Degree in Social Work (CSWE accredited) and two years paid post-degree experience in coordinating services.
• Bachelor’s or Master’s Degree in Nursing (RN) with Louisiana license and two years paid post-degree experience in coordinating services.
• Bachelor’s or Master’s Degree in a Human Service related field and two years paid post-degree experience in coordinating services.
• Bachelor’s Degree in Liberal Arts with 16 hours in psychology, education, counseling or related fields and two years post-degree experience in coordination services.
• Reliable transportation for field visits (home visits about 50% or more of the time).
• Easterseals Louisiana is a Drug‑Free Workplace; pre-employment and random drug tests will be administered.
• Must pass Easterseals Louisiana’s background check and driver’s license check.

Responsibilities

• Complete quarterly visits, generating required units, observation and monitoring, and monthly contacts.
• Conduct and complete annual CPOCs per funder timelines.
• Plan, schedule, and coordinate quarterly meetings with participants and providers.
• Respond to participant changes in needs, completing/submitting appropriate paperwork.
• Research, access, and link services.
• Communicate with providers to ensure appropriate service provision.
• Maintain responsibilities and requirements regarding critical incidents.
• Meet with supervisor for weekly supervision and participant needs.
• Attend weekly staff meetings and required training.
• Support participants during emergent situations and weather-related evacuations.
• Adhere to mandated reporter requirements per Louisiana Children’s Code.

Education

Bachelor’s or Master’s Degree in Social Work (CSWE accredited)
