Job summary

Easterseals Louisiana seeks Support Coordinators (case managers) to serve individuals with intellectual and developmental disabilities. You will plan, organize, implement, and monitor in-home and community-based services for a caseload of 35-45 participants living in their homes and communities.

Responsibilities include quarterly visits, coordinating meetings, responding to changes, researching services, and maintaining safety protocols.

Qualifications

• Bachelor’s or Master’s Degree in Social Work (CSWE-accredited) with two years of post-degree coordination experience.
• Bachelor’s or Master’s Degree in Nursing (RN) licensed in Louisiana with two years post-degree coordination experience (one year can substitute).
• Bachelor’s or Master’s Degree in a Human Service field with two years post-degree coordination experience.
• Bachelor’s Degree in Liberal Arts with 16 hours in relevant fields and two years post-degree coordination experience.

Responsibilities

• Complete quarterly visits and monthly contacts.
• Conduct annual CPOCs according to funder timelines.
• Plan and coordinate quarterly meetings with participants and providers.
• Respond to participant changes and submit appropriate paperwork.
• Research and link services.
• Coordinate with providers to ensure services.
• Maintain obligations regarding critical incidents.
• Weekly supervision and participant need reviews.
• Attend weekly staff meetings and required training.
• Support participants during emergent situations and evacuations.

Education

Bachelor’s or Master’s Degree in Social Work (CSWE-accredited) and two years of paid post-degree coordination experience

Bachelor’s or Master’s Degree in Nursing (RN) licensed in Louisiana and two years of paid post-degree coordination experience

Bachelor’s or Master’s Degree in a Human Service related field and two years of paid post-degree coordination experience

Bachelor’s Degree in Liberal Arts with 16+ hours in psychology/education/counseling/social services etc. and two years of coordination experience
