Job summary

The Brigham and Women's Hospital seeks a Community Resource Specialist to join the Substance Use Disorder Bridge clinic. You will help patients access community resources for SUD treatment, mental health services, housing, food, and transportation, interfacing with a multidisciplinary team to ensure timely support and coordinated care.

Qualifications

• 2 years' work experience in the substance use disorder field preferred.
• Strong interpersonal and communication skills.
• Flexible to adapt to program evolution and shifts in prioritization.
• Ability to work independently with minimal supervision and as part of a team.
• Demonstrated organizational and time management skills.
• Ability to learn new technology systems for patient documentation.

Responsibilities

• Assist patients in finding community resources for SUD treatment and social determinants of health.
• Research resources and communicate results to the care team and patients when indicated.
• Upload/refers sources to the resource database and maintain it.
• Coordinate referrals to internal and external resources and triage service requests.
• Collaborate with Bridge clinic physicians, therapists, recovery coaches, and care staff.
• Document encounters, resources, and communications in EHRs and assist in patient tracking.
