Job summary

The Brigham and Women's Hospital, Inc. is seeking a Community Resource Specialist to join the Substance Use Disorder (SUD) Bridge clinic on the Mass General Brigham network.

You will work with physicians, therapists, and recovery coaches to connect patients with community resources and coordinate referrals. This on-site role requires strong communication, attention to detail, and the ability to navigate resources for patients with diverse needs, including social determinants of health.

Qualifications

• High school diploma or equivalent required; associate degree preferred.
• 2 years' work experience, preferably in the substance use disorder field.
• Strong interpersonal and communication skills.
• Flexibility to adapt to program evolution and shifts in prioritization.
• Ability to work independently with minimal supervision and as part of a team.
• Demonstrated organizational and time management skills.

Responsibilities

• Assist in identifying accessible community resources for SUD treatment and social determinants of health.
• Coordinate referrals to internal and external resources.
• Collaborate with Bridge clinic physicians, clinical therapists, and recovery coaches.
• Track and document encounters, resources provided, and case-related communications in the EHR.
• Assist in maintaining patient tracking database and provide coverage as needed.
