Job summary

• Conduct individual and group counseling sessions for clients and families.
• Provide relapse prevention education and coping skills training.
• Coordinate care with referral sources, family members, and involved organizations to enhance outcomes.
• Maintain documentation and progress reports in the electronic health record system.
