Job summary

Eliot Community Human Services, Inc. is seeking a Substance Abuse Case Manager to join our PACT team. The role emphasizes primary case management, individualized substance use treatment, and recovery‑oriented care for adults with serious mental illness.

The ideal candidate will facilitate dual diagnosis groups, provide integrated treatment, and participate in assessments with the team. Schedule includes rotating on-call coverage and on-site work in Massachusetts.

Qualifications

• Bachelor's degree in Psychology, Social Work or related field is required.
• Experience working with adults with substance abuse disorders is required.

Responsibilities

• Act as primary case manager for identified clients, providing targeted support and interventions.
• Provide individual and group substance use treatment and support.
• Participate in assessments and treatment planning with clients and the team.
• Ensure consumer safety by implementing crisis intervention techniques when necessary.
• Utilize a team based approach to support and services, collaborating with colleagues and actively participating in daily meetings.
• Participate in rotating on-call coverage and evening shifts to ensure 24/7 support for clients.
