Job summary

Vermont State Colleges System is seeking a Health & Wellness Case Manager to support students facing complex challenges, coordinating campus and community resources. The role focuses on crisis response, medical/mental health leave processes, and re-entry planning, within a student-centered framework.

Responsibilities include managing a caseload, conducting assessments, and collaborating with CARE teams and campus partners to promote student success and retention across Vermont.

Qualifications

• Bachelor's degree in counseling, social work, higher education, public health, or related field, plus one year of relevant experience.
• Demonstrated ability to manage sensitive situations with professionalism, discretion, and sound judgment.

Responsibilities

• Manage a caseload of students with elevated or complex concerns (mental health, medical, academic, personal).
• Conduct individualized assessments and develop tailored intervention and support plans.
• Provide ongoing case management with regular follow-ups, monitoring, and documentation.
• Facilitate referrals to campus and community resources for coordinated support.
• Support students in crisis, including hospitalization or mental health emergencies.
• Assist students navigating medical leave and return-to-campus processes.
