Job summary

Southeast Community College - Milford Campus is seeking a Student Care Case Manager to provide holistic, student-centered support for students facing personal, mental health, academic, financial, housing, or social challenges.

The role requires strong communication and collaboration with the CARE Team, CAPS program, and campus partners to assess needs, develop tailored intervention plans, and navigate complex considerations while documenting progress and outcomes.

Qualifications

• Bachelor's degree in Social Work, Counseling, Psychology, Human Services, or a closely related field.
• Two or more years of experience in de-escalation, behavior intervention, and/or case management.

Responsibilities

• Provide direct outreach and case management to distressed students referred to CARE Team.
• Assess student strengths, risks, and barriers to tailor intervention strategies.
• Develop, monitor, and evaluate intervention plans with campus partners.
• Coordinate safe, timely, effective, equitable, student-centered care.
• Serve as liaison between students, faculty, staff, and families.
• Communicate case updates and outcomes while maintaining confidentiality.
• Document CARE Team activities and outcomes in campus systems.
