Community Health Centers of Benton and Linn Counties (CHC) is seeking a Primary Care Referral Coordinator to process internal referrals to outside providers and specialists. You will understand medical needs, document actions, and communicate with patients while documenting work in the electronic health record.

Qualifications

• High school diploma or GED; AND 4 years of administrative experience including office support duties, reception, clerical functions or related experience.
• Equivalent combination of education and experience may be accepted.
• Candidates must have at least 6 months of actual work experience to be considered as equivalent.

Responsabilités

• Receives and processes internal referrals from the care team to outside providers and specialists.
• Documents the medical need, action requested, and insurance considerations.
• Communicates with patients and documents work in the electronic health record.
