Job summary

Outfield Healthcare Partners is seeking an experienced MDS Coordinator to oversee timely completion of the MDS process and care management from admission to discharge, aligned with policy and regulatory guidelines.

The role requires RN licensure, knowledge of Medicare PPS, and familiarity with the RAI User's Manual, OBRA, and MDS 3.0. Collaboration with the DON and VP of Clinical Reimbursement is essential for accurate data and compliant documentation.

Qualifications

• RN licensed in the state of practice.
• Minimum 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Thorough understanding of OBRA regulations and MDS processes.
• Experience with MDS 3.0 preferred.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the Interdisciplinary Team.
• Ensure accurate MDS coding and documentation in the medical record.
• Monitor and update resident data in MDS programs.
• Prepare and communicate scheduling for care planning conferences.
• Maintain confidentiality and compliance with federal/state regulations.
