Job summary

Outfield Healthcare Partners seeks an experienced MDS Coordinator to manage the RAI and care management processes from admission to discharge, in accordance with policy and Federal, State and Certification guidelines and applicable regulations.

Under direction of the Director of Nursing and VP of Clinical Reimbursement, you will coordinate information systems, education for the clinical department, maintain MDS status, ensure accurate coding, and support the interdisciplinary team.

Qualifications

• Graduate of an approved RN program with active license in Missouri.
• Minimum 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Thorough understanding of MDS, PPS, and Medicaid reimbursement.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the Interdisciplinary Team.
• Ensure accurate MDS coding and documentation in medical records.
• Monitor MDS status and data entry into appropriate systems.
• Prepare and schedule resident care planning conferences and communicate outcomes.
• Comply with federal and state regulations regarding the RAI process.
