Job summary

OPCO Skilled Management seeks an MDS Coordinator to support the Director of Nursing and the RN Assessment Coordinator in ensuring that documentation meets Federal, State, and Certification guidelines. The role coordinates the RAI process including MDS, CAAs, and the interdisciplinary care plan.

This position supports timely submissions, accuracy checks, and data tracking within PointClickCare, working with staff to uphold high standards of practice and regulatory compliance.

Qualifications

• Graduate of an approved Registered Nurse / Licensed Vocational Nurse program and licensed in the state of practice required.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred; knowledge of Case-Mix, Medicare PPS, Medicaid reimbursement, OBRA, MDS and care planning.

Responsibilities

• Assure adherence to Federal and State regulations and certification.
• Participate in regulatory surveys and correction of deficiencies.
• Report trends from audits to the Quality Assurance Committee.
• Ensure timely completion of the RAI process from MDS to care plan.
