Job summary

SNF Staffing Solutions is seeking a Registered Nurse to serve as an MDS Coordinator, responsible for timely and accurate completion of the RAI process and care management from admission to discharge, per policy and applicable guidelines.

Under the direction of the Director of Nursing and VP of Clinical Reimbursement, you will coordinate data entry, assess residents with the Interdisciplinary Team, and ensure compliance with state and federal regulations.

Qualifications

• RN licensed in Texas and graduate of an approved nursing program.
• Strong knowledge of MDS 3.0 and related Federal Medicare PPS processes.
• Experience with OBRA regulations and Minimum Data Set.
• Ability to coordinate care with an Interdisciplinary Team.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the Interdisciplinary Team.
• Ensure MDS assessments follow RAI guidelines and governing agency criteria.
• Schedule resident care planning conferences and communicate outcomes to staff and families.
• Maintain accurate MDS status and documentation to support coding.
• Submit electronic documentation to state databases as required.
