Job summary

OPCO Skilled Management seeks an MDS Coordinator to ensure timely completion of the RAI process and care management from admission to discharge in compliance with state and federal guidelines.

The ideal candidate is a licensed RN or LVN with experience in Skilled Nursing Facility MDS 3.0, Medicare PPS, and Medicaid reimbursement, and can coordinate interdisciplinary care while maintaining accurate documentation.

Qualifications

• RN or LVN licensed in the state.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Knowledge of MDS, RAI guidelines, and state/federal regulations.
• Experience with MDS 3.0 preferred.
• Understanding of OBRA regulations.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Ensure decisions about MDS are based on clinical criteria per the RAI User’s Manual.
• Coordinate daily stand-up meetings and set assessment reference dates.
• Comply with federal and state regulations regarding MDS completion and coordination.
• Monitor MDS and care plan documentation; ensure accuracy in medical records.
• Maintain current MDS status per guidelines.
• Maintain data entry of resident information into MDS programs.
• Code the MDS accurately from chart review and interviews.
• Attend interdisciplinary team and QA meetings to update records.
• Assist DON with identification of significant changes and communication of changes.
• Prepare scheduling and notices for care planning conferences.
• Attend weekly Medicare, Medicaid, and Managed Care meetings.
• Submit final MDS data to state and federal agencies.
• Coach staff on RAI, PPS, and clinical systems.
