Job summary

Advanced Center for Nursing and Rehabilitation is seeking a qualified MDS Coordinator to manage the RAI/MDS process in a skilled nursing environment. You will coordinate assessments, care plans, and regulatory compliance to ensure high-quality resident care and optimized reimbursement.

The role requires RN licensure with knowledge of MDS 3.0, RAI, and PDPM. Strong organizational and communication skills are essential for collaborating with nursing, therapy, and billing teams.

Qualifications

• RN licensed, with active registration.
• RAC-CT certification preferred.
• 1–3 years in long-term care, skilled nursing, or rehabilitation.
• Strong knowledge of MDS 3.0, RAI, CMS regs, and PDPM.
• Proficiency with electronic health records (EHR).
• Excellent organizational, analytical, and communication skills.

Responsibilities

• Coordinate and complete MDS assessments per regulatory timelines.
• Develop, coordinate, and update individualized care plans.
• Maintain CMS/state regulatory compliance and participate in surveys/audits.
• Support Medicare/Medicaid reimbursement through accurate coding and documentation.
• Analyze quality indicators and participate in performance improvement.
