Job summary

Care Initiatives seeks an Admissions Coordinator to drive census growth and optimize payer mix by building strong referral relationships and managing the admissions process end-to-end. You will coordinate with discharge planners, central intake, and business office to secure timely, compliant placements.

The role blends outside sales, account management, and admissions operations to deliver high-quality intake experiences for residents and families. Prior healthcare exposure is helpful.

Qualifications

• Associate or bachelor’s degree in healthcare, business, marketing, or related field preferred.
• 2+ years in post-acute admissions, healthcare sales, or discharge planning is a plus.
• Experience with referral management systems and Microsoft 365 is beneficial.

Responsibilities

• Develop market coverage with hospitals, SNFs, and other care providers.
• Manage admissions pipeline end-to-end and ensure timely placements.
• Collaborate with care teams to verify insurance benefits and authorizations.
• Maintain and analyze referral source plans and achieve targets.
• Navigate HIPAA compliance, patient privacy, and regulatory standards.
• Present capabilities and admit criteria to planners and teams.
