Qualifications

• Associate or bachelor's degree in healthcare, business, marketing, social work, or related field.
• Two or more years in post-acute admissions, healthcare sales, case management, or discharge planning; LTC experience preferred.
• CRM proficiency and Microsoft 365 familiarity required.

Responsibilities

• Develop and execute market coverage to hospitals, SNFs, assisted living, home health, and physician practices.
• Maintain account plans; identify decision makers and barriers; set SMART goals.
• Present capabilities, bed availability, and admit criteria; conduct in-service education and reviews.
• Manage CRM; log activities, track stages, and report pipeline weekly.
• Verify insurance/benefits with central intake and accelerate authorizations.
• Analyze trends and adjust outreach to hit targets.
• Triage referrals; ensure complete files; coordinate assessments with nursing/therapy.
• Guide residents and families through benefits, paperwork, and financial discussions.
