Qualifications

• Coordinate home health aides' schedules daily.
• Field patient/customer inquiries by phone with knowledge of processes (CICOA).
• Monitor and log patient, client, and aide activity in system.
• Follow up on home health assignments and patient services.
• Maximize utilization of waiver-related hours and prior authorizations.

Responsibilities

• Coordinate home health aides' schedules on a daily basis.
• Monitor, mediate, and log patient/client and home health aides in the system.
• Follow up on home health assignments and patient/client services.
• Ensure optimal utilization of CICOA, WAIVER, VA, and Prior Authorization hours.
• Participate in on-call rotation to support aides and residents.
