Job summary

LHH in San Jose, CA is seeking a compassionate Case Manager to support individuals experiencing homelessness in achieving housing stability and self-sufficiency. You will conduct intakes, assessments, and service planning, connecting clients to housing, healthcare, behavior health, employment, and community resources.

The role requires at least 1 year in case management or related fields, a valid driver's license, and the ability to pass background checks and TB tests.

Qualifications

• 1 year of experience in case management, homeless services, healthcare, behavioral health, social services, or related field.
• Passion for serving vulnerable populations and supporting housing stability.
• Valid driver's license, reliable transportation, and auto insurance.
• Ability to pass background check, drug screen, and TB test; CPR/First Aid certification or willingness to obtain.

Responsibilities

• Conduct client intakes, assessments, and service planning.
• Connect clients to housing, healthcare, behavioral health, employment, and community resources.
• Provide ongoing case management, crisis intervention, advocacy, and housing support.
• Maintain accurate case notes, client records, and program documentation.
• Utilize motivational interviewing, trauma-informed care, and harm reduction practices.
• Collaborate with community partners and multidisciplinary teams.
