Job summary

Sweetwater Care in the United States is seeking a Speech-Language Pathologist to support patients with speech, language, swallowing and cognitive disorders in a SNF setting. The role emphasizes evaluation, treatment planning, and progress documentation to improve communication and swallowing safety.

You will collaborate with rehab team members, develop plans of care, obtain physician approvals, and ensure compliance with Medicare/Medicaid and company policies.

Responsibilities

• Direct all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation according to company guidelines, professional standards and community needs.
• Provide professional treatment using therapeutic techniques for rehabilitation of speech, language, hearing, oral motor, swallowing and cognitive disorders.
• Plan activities and provide therapeutic services for speech and language disorders.
• Serve as patient and rehabilitation advocate; participate in quality assurance and customer service activities.
• Perform patient evaluations and develop treatment plans with short- and long-term goals per physician orders.
• Document evaluations, progress notes, discharge summaries for medical records timely.
• Demonstrate knowledge of Medicare, Medicaid and other reimbursement systems.
• Develop and obtain approval for services from referring physicians.
• Collaborate with other therapists and physicians on patient treatment programs and progress.
• Ensure delivery of quality rehab services and maintain courtesy and respect toward patients and staff.
• Maintain compliance with the Compliance Program and Code of Conduct.
