Job summary

Sweetwater Care is seeking a licensed speech-language pathologist to provide comprehensive therapy for patients with speech, language, swallowing and cognitive disorders in a SNF setting. You will evaluate, plan, implement and document care while coordinating with physicians and therapists to optimize patient outcomes.

Responsibilities include developing treatment plans, securing approvals, and ensuring compliance with reimbursement guidelines and company policies.

Qualifications

• Evaluation and treatment planning for speech, language, swallowing, and cognitive disorders.
• Document progress and outcomes per department policy and physician orders.
• Knowledge of Medicare/Medicaid reimbursement and related processes.
• Ability to develop and adjust plans of care with physician approval.
• Collaborate with multi-disciplinary teams to support patient needs.

Responsibilities

• Direct all aspects of quality patient care including evaluation, treatment and documentation.
• Provide professional treatment for speech, language, swallowing, and cognitive disorders.
• Plan activities and services for speech and language therapy.
• Collaborate with physicians, therapists and other healthcare team members.
• Prepare progress notes, discharge summaries and Plan of Care updates.
• Ensure delivery of rehab services in accordance with policy and standards.
• Maintain compliance with the organization’s Code of Conduct.
