Job summary

Sweetwater Care is seeking a Speech-Language Pathologist to assist patients with communication, swallowing and cognitive disorders in a skilled nursing facility setting. You will assess, treat and document progress, collaborate with a multidisciplinary team and ensure adherence to physician orders and care plans.

The role emphasizes rehabilitation services, patient advocacy, and coordination with physicians, families and care teams to optimize outcomes in a SNF environment.

Responsibilities

• Direct all aspects of patient care, including evaluation, treatment planning, goal setting, family education and documentation.
• Provide professional speech-language therapy services for rehabilitation of communication, swallowing and cognitive disorders.
• Plan activities and deliver services for speech and language disorders.
• Act as patient advocate and participate in quality assurance and customer service activities.
• Perform evaluations and develop treatment plans aligned with physician orders and patient condition.
• Progress patients within the established care plan and document outcomes.
• Demonstrate knowledge of Medicare, Medicaid and other reimbursements.
• Develop treatment plans and obtain approvals from referring physicians.
• Collaborate with other therapists and physicians on patient treatment programs.
• Document evaluations, progress notes and discharge summaries in patient records.
• Provide a full range of speech therapy modalities and supervise therapy assistants as needed.
• Ensure quality rehab services and maintain courtesy, compassion and respect for patients and staff.
• Comply with the company Code of Conduct and assist with case management needs.
