Job summary

Sweetwater Care is seeking a Speech-Language Pathologist to provide comprehensive services for patients with communication, swallowing and cognitive disorders in a SNF environment. The role involves evaluation, treatment planning, and collaboration with a multidisciplinary team to optimize patient outcomes.

The successful candidate will perform assessments, develop and monitor treatment plans, document progress, and ensure compliance with regulatory standards while delivering compassionate care

Responsibilities

• Direct all aspects of quality patient care, including evaluation, treatment and documentation per guidelines and standards.
• Provide professional speech-language pathology services for rehabilitation of speech, language, hearing, oral‑motor, swallowing and cognitive disorders.
• Plan activities and provide therapy for speech and language disorders.
• Advocate for patients; participate in quality assurance and customer service activities.
• Evaluate patients and develop treatment plans with short‑/long‑term goals as ordered by physicians.
• Implement care plans and adjust progression within the established plan.
• Understand Medicare, Medicaid and other reimbursement systems.
• Develop treatment plans and obtain approval from referring physicians.
• Participate in patient care planning and utilization reviews.
• Provide patient care appropriate for SNF settings and administer standardized assessments and swallowing evaluations.
• Conduct tests and complex treatments individually or with support.
• Collaborate with occupational, physical therapists and physicians on treatment plans.
• Document evaluations, progress notes and discharge summaries timely.
• Deliver a full range of speech therapy modalities.
• Assist with clinical affiliations of speech therapy assistants.
• Assess patient needs and determine appropriateness of care as ordered by physician.
• Prepare Plan of Care, progress notes and other clinical records regularly.
• Ensure delivery of quality rehab services.
• Demonstrate courtesy, compassion and respect to patients and staff.
• Maintain compliance with the Compliance Program and Code of Conduct.
• Coordinate with facility and other organizations on case management needs.
• Assume additional duties as required.
