Job summary

Sweetwater Care is seeking a Speech-Language Pathologist to assist patients with speech, language, swallowing, and cognitive disorders in a SNF setting. You will conduct assessments, develop treatment plans, and document progress while collaborating with physicians, therapists, and caregivers.

Responsibilities include evaluating patients, planning therapy, and ensuring high-quality rehab services in alignment with physician orders and Medicare/Medicaid guidelines.

Qualifications

• Must provide speech-language pathology services in a SNF setting.
• Ability to perform assessments and develop treatment plans.
• Experience with Medicare/Medicaid and other reimbursement systems is preferred.

Responsibilities

• Direct all aspects of quality patient care including evaluation, treatment, planning and documentation.
• Provide professional treatment using speech, language, hearing, oral motor, swallowing and cognitive therapies.
• Develop and implement treatment plans and goals.
• Document evaluations, treatment progress, and discharge summaries per policy.
• Collaborate with other health professionals on patient care plans.
