Sweetwater Care is seeking a Speech-Language Pathologist to provide comprehensive evaluation, treatment planning and therapy for patients with speech, language, swallowing and cognitive disorders in a SNF setting. Responsibilities

• Direct all aspects of quality patient care, including evaluation, treatment planning, goal setting, and documentation.
• Provide professional speech-language therapy for patients with speech, language, hearing, swallowing, and cognitive disorders.
• Plan activities and provide therapeutic services for speech and language disorders.
• Advocate for patients and participate in quality assurance and customer service activities.
• Perform evaluations and develop treatment plans with short- and long-term goals per physician orders.
• Carry out care plans and determine patient progression within the established plan.
• Demonstrate knowledge of Medicare, Medicaid and other reimbursement systems.
• Develop treatment plans and obtain physician approval.
• Participate in care planning and utilization review meetings.
• Provide appropriate SNF patient care knowledge and skills.
• Administer standardized speech/language assessments and swallowing evaluations; monitor progress.
• Perform tests and complex treatments individually or with assistance.
• Collaborate with occupational and physical therapists, physicians, and other team members.
• Document evaluations, progress notes, and discharge summaries promptly per policy.
• Provide a full range of speech therapy modalities.
• Assist with clinical affiliations of speech therapy assistants.
• Perform clinical assessments to identify patient needs and administer physician-ordered care.
• Document and maintain Plan of Care, progress notes, and orders.
• Ensure delivery of quality rehab services.
• Show courtesy, compassion, and respect to patients and team.
• Maintain compliance with the program and code of conduct.
• Collaborate on case management needs with facilities and organizations.
• Adhere to evolving duties as assigned.
