Responsibilities

• Direct all aspects of patient care including evaluation, treatment planning, goal setting, family education and documentation.
• Provides professional treatment for rehabilitation of speech, language, hearing, oral motor, swallowing and cognitive disorders.
• Plans activities and provides therapeutic services for speech and language disorders.
• Develops treatment plans and obtains approval from referring physicians.
• Performs patient evaluations and develops plans of care with short- and long-term goals.
• Collaborates with other therapists and physicians to coordinate patient care.
• Documents evaluations, progress notes, discharge summaries and related records.
